Ensuring all Needs are Met.

The assessment may be your first opportunity to meet and interact with our Executive Director, who is also the building’s on-site Nurse. The assessment helps create a service plan for Resident Care staff to follow. Our goal is to ensure all resident needs – physical, social, medical, and psychological – are met. This plan is fluid and will adapt as your needs change.

WHAT IS THE ASSESSMENT?

Our Executive Director will ask a series of questions relating to your daily activities and personal hygiene and habits. This will help us better understand what assistance you may need on a daily basis.

The Executive Director will also perform a cognitive exam. It’s important to remember, this only establishes a baseline for staff to fully understand how you currently process information.
